Transferring Your Medical Records
To make your first appointment go as smoothly as possible, we will need your medical records ahead of time. Here’s how to send them over:
- Fill out our Authorization to Disclose Information form.
- Email the completed form to macroi@mahealthcare.com.
Important: Please send your form directly to our Medical Records department to ensure your health information is imported into your electronic medical record for quick provider access before your first visit.
