To explain my "no" answer to the first question: I was very sick when I called on a Thursday for an appointment. Coughing, raw, infected throat, and could barely talk. I know from past experience that Dr would have worked me in. But receptionist looked at schedule and said she couldn't get me in today (Thu) or Friday. That meant going 4 days without being seen.
