German Lutheran women of a certain generation will only answer that question with "I'm fine." It's cultural. Even if they feel like keeling over, they are "fine, thank you." In my mother's case, questions like "Have you felt dizzy lately?" "Do you have headaches very often?" How does your head feel when you stand up?" These types of questions would generate a more accurate response."
