Disease Management The Quality Improvement Department has two active disease management programs. These programs are Diabetes and Hypertension. The purpose of disease management is to provide members with educational materials and support to help them deal effectively with their illness. The program includes patient education, guidelines for care, recommended lab schedules, and phone contacts with the designated disease management nurse. Through education and monitoring, we attempt to effectively promote patient compliance and healthy lifestyles. Medical Associates Health Plans is continually striving to improve the quality of care and services provided to its members through efforts aimed at Disease Management. The goals are to reduce/eliminate hospital admissions, promote wellness, and encourage an active participation from the member.
