Patient Rights & Responsibilities

Your Rights as a Patient of Medical Associates Clinic

  1. You have the right to voice complaints or appeals about the Medical Associates Health Plan or care provided.
  2. You have the right to participate with practitioners in making decisions about their health care.
  3. You have the right to be treated with respect and recognition of their dignity and right to privacy.
  4. You have the right to receive information about the MAHP, services, practitioners providing care, and Members’ rights and responsibilities.
  5. You have the right to a candid discussion of appropriate or medically necessary treatment options for their conditions, regardless of cost or benefit coverage.
  6. You have the right to make recommendations regarding the organization’s member rights and responsibilities policies.

Your Responsibilities as a Patient of Medical Associates Clinic

  1. You are expected to supply information (to the extent possible), that the MAHP and its practitioners and providers need in order to provide care.
  2. You are expected to follow Medical Associates Health Plans and instructions and guidelines for care that they have agreed on with their practitioners.
  3. You are expected to understand their health problems and participate in developing mutually agreed upon treatment goals to the degree possible.
